Key Responsibilities

• Provide holistic patient care that honors each patient's rights and dignity. Understand and integrate individual values, preferences, and cultural backgrounds into your nursing practice.
• Conduct thorough assessments of patients’ physical, psychological, and social needs upon admission and throughout their stay.
• Monitor and manage pain levels of patients, implementing effective pain management strategies and adjusting care plans appropriately.
• Develop, implement, and regularly update individualized care plans based on patient diagnosis, condition changes, and established standards of care.
• Actively involve patients and their families in care decisions, ensuring they are informed and comfortable with their care plans.
• Document all patient interactions, assessments, treatments, and outcomes meticulously, adhering to HIPAA regulations and facility policies.
• Review patient medical histories, lab results, and diagnostic tests, quickly acting upon any abnormalities by communicating findings to the appropriate medical staff.
• Educate patients and their families on managing illnesses and injuries, including discharge instructions and follow-up care to foster long-term health.
• Administer medications, treatments, and interventions as prescribed while monitoring patients for reactions and side effects.

Patient Population / Client Focus

You will primarily care for patients with various acute and chronic conditions, with a special focus on cardiovascular health. The diverse patient population includes elderly individuals recovering from surgeries, residents with chronic illnesses, and those requiring long-term rehabilitation. Your role will be pivotal in promoting health education, disease prevention, and encouraging a healthy lifestyle to improve overall well-being.

Qualifications

• Must possess a Bachelor’s Degree in Nursing from an accredited institution.
• Current, valid Registered Nurse (RN) license in the state of Illinois is required.
• Current certification in CPR is mandatory.
• Demonstrated excellent communication and interpersonal skills, with the ability to maintain professionalism in high-pressure situations.
• Strong ability to apply critical thinking and problem-solving skills effectively.

Compensation and Benefits
