Work Environment

You will be working onsite at our state-of-the-art hospice center located in Lorain, Ohio, a community known for its welcoming spirit and commitment to compassionate care. Key Responsibilities

• Conduct comprehensive initial assessments of patients to evaluate their medical and emotional needs, formulating individualized care plans in collaboration with the caregiving team.
• Administer prescribed medications and treatments, consistently monitoring for efficacy and patient reactions, and adjusting care plans accordingly.
• Regularly assess and document patients’ vital signs, symptoms, and medical history, ensuring meticulous record-keeping that facilitates coordinated care.
• Educate patients and family members about treatment options, medications, and effective disease management strategies, fostering empowerment through knowledge.
• Assist physicians and specialists during medical procedures, ensuring a safe, comfortable environment for patients while adhering to safety protocols.
• Respond promptly to medical emergencies, utilizing critical thinking and established protocols to ensure patient safety and comfort.
• Maintain accurate documentation in patient medical records to guarantee continuity of care and legal compliance across all facets of care delivery.

Patient Population / Client Focus

In this role, you will primarily work with patients facing terminal illnesses who require specialized hospice care. These patients, along with their families, are experiencing one of the most challenging times of their lives. You will provide not only medical care but also emotional support, allowing you to create deep connections and fulfill their unique needs at a vulnerable stage.

Qualifications

• An active and valid Registered Nurse license, eligible for practice in Ohio.
• A Bachelor's Degree in Nursing (BSN) is required; applicants who do not yet have a BSN but are pursuing one will also be considered.
• A minimum of one year of nursing experience, preferably within a clinical or hospital setting, demonstrating a well-rounded background and capability.
• Proficient verbal and written communication skills, enabling you to effectively interact with patients, families, and team members.
• Strong ability to work in fast-paced settings, embrace change, and handle multiple priorities simultaneously.
• A willingness to work flexible hours, including weekends and holidays, contributing to a collaborative care schedule.

Preferred Experience

• Prior experience in hospice care or palliative care environments is highly desirable.
• Familiarity with advanced care planning, symptom management, and end-of-life discussions is beneficial in this role.
• Proven ability to utilize electronic health record (EHR) systems for documentation and healthcare management is preferred.

Compensation and Benefits

• Competitive annual salary ranging from $70,000 to $160,000, commensurate with experience, including routine performance evaluations.
