What You'll do:
As a Home Health RN, you'll deliver skilled, individualized nursing care to patients in the comfort of their own homes- coordinating care, building trust , and making a measurable impact every day.
Key Responsibilities
Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
Responds promptly to emails and phone calls
What we are looking for:
Current RN license in the state of Massachusett
Strong technology skills
Reliable transportation, valid driver's license, auto insurance
