Job Overview
A highly respected membership-based internal medicine practice that has provided continuous, patient-centered care in the Fort Worth medical district for over 40 years. We are seeking a highly experienced, professional, and collaborative Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our tight-knit team.
Unlike traditional high-volume clinics, this role does not focus on standard floor tasks like rooming patients or checking vital signs, which are already handled by our dedicated clinical staff. Instead, you will serve as a premier clinical collaborator, focusing directly on patient education, advanced care coordination, and proactive practice improvement to uphold our core values of professionalism, respect, and cordiality.
Key Responsibilities
· Patient Education & Communication: Deliver expert clinical advice regarding laboratory and test results via telephone and email.
· Referral Management & Continuity: Compose clear, professional referral letters to specialists (e.g., dermatologists for cancer screenings, cardiologists for atrial fibrillation management) and bundle them seamlessly with corresponding chart notes.
· Care Coordination: Manage the medical insurance preauthorization process to ensure timely access to prescribed patient care and medications.
· Practice Collaboration: Act as a core collaborator alongside the physicians to identify operational bottlenecks, improve clinical processes, and address unmet patient needs.
Position Details & Benefits
· Schedule & Hours: Full-time, 5 days per week (Minimum of 40 hours per week). Hours are flexible to accommodate practice flow and personal balance.
· Work Environment: Located in a secure, small medical office facility just two blocks from Harris Hospital. The facility features monitored security cameras inside the office and across the parking lot to maintain our historically safe environment.
Qualifications and Skills
· Licensure: Active, unencumbered Texas state nursing license as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN).
· Experience: High level of prior nursing experience, preferably in outpatient internal medicine, care management, or triage settings.
· Skills: Exceptional written communication skills for professional correspondence, strong critical thinking, and comfort using email and Electronic Health Record (EHR) platforms.
