Job Description
Qualifications:

  • High school diploma or equivalent diploma
  • Certified Nursing Assistant certification
  • 1 year or more clinical, bedside CNA experience
  • Able to read, write, and speak effectively
  • Able to type and use computer equipment and software
  • Good physical health condition and pass a physical examination
Other duties may be assigned:
  • Provides and assists with restorative programs:
  • ROM
  • Ambulation
  • Bed mobility, positioning and transfer techniques
  • Application of splints, braces, cones, prosthetic devices and care of limbs where applied
  • Moist heat therapy
  • Assistive feeding devices and restorative dining techniques; cueing and swallowing
  • Activities of daily living; bathing, dressing, grooming, personal hygiene
  • Incontinence management
  • Transferring
  • Communication
  • Participates in, supports and adheres to the philosophies of the restorative nursing program as provided by the restorative nurse
  • Participates in restorative dining program, directs regular CNAs about resident restorative dining programs
  • Serves as a resource and role model for CNAs
  • Documents program completion every day and narrative progress notes as required by facility policy
  • Attends restorative meetings and care plan conferences as required
  • Reports unusual problems regarding resident care to nursing staff immediately: pain, refusals, falls, etc.
  • Reports malfunctioning or improperly fitting equipment and equipment needs to therapy and restorative nurse
  • Reinforces team management of bowel and bladder program and works with CNA on B/B procedures/program
  • Offers suggestions and provides input into development of interdisciplinary plan of care as required
  • Assists with general orientation and on-going restorative nursing inservices as required
  • Attends required nursing inservices
  • Participates in the facility Quality Assurance program as directed by the restorative nurse, and reports areas needing improvement
  • Keeps restorative knowledge and skills current by attendance in inservices and seminars as required
  • Performs therapy delegated tasks under the direction and supervision of therapy as required
  • Answer call lights to determine residents' needs.
  • Serve and collect food trays, feed residents requiring help, and provide between meal nourishment, ice, and fresh drinking water as required
  • Transport residents using wheelchairs, geri-chairs, lifts, and other assistive devices.
  • Ensure that resident's rights are honored including but not limited to personal choices, privacy, and confidentiality.
  • Report all changes in conditions and behaviors of residents to the Director of Nursing or the RN in charge.
  • Promptly report all incidents involving residents, staff, families, and visitors to the Director of Nursing or the RN in charge.
  • Maintain infection control standards by use of universal precautions.
  • Follow facility policies and procedures.
  • The Associate is authorized to have limited access to resident health information as needed to provide treatment, for billing purposes, or for the purpose of healthcare operations.
  • Other duties/tasks as assigned
