Job Title: Remote RN Case Manager-Bilingual in Spanish
Duration: 6 Months (Extension Possible)
Pay Rate: $48/hr (W2)
Location: 100% Remote (Open to all U.S. time zones; CST preferred for 1–2 candidates)
Work Authorization: Must be a U.S. Citizen
Job Overview:
We are seeking a skilled Remote RN Case Manager to join our team on a 6-month contract, with potential for extension. This role is focused on ensuring that members with complex medical or psychosocial needs receive high-quality, cost-effective care. The RN Case Manager will perform holistic assessments and collaborate with internal and external stakeholders to develop and implement individualized care plans.
Key Responsibilities:
- Conduct telephonic interviews using motivational interviewing techniques
- Collaborate with case management team, physicians, and interdepartmental staff
- Educate members using approved tools to support care plan adherence
- Collect and evaluate medical data to identify unmet needs
- Create and monitor individualized care plans
- Coordinate with providers, home health, and community resources
- Participate in interdisciplinary team meetings and quality initiatives
- Maintain accurate documentation and adhere to regulatory guidelines
- Remain informed about benefit design, cost analysis, and community resources
- Promote collaboration with external payers, agencies, and partners
- Participate in internal training and continuing education
Required Qualifications:
- RN License with Bachelor’s degree in Nursing (or equivalent RN certification)
- Minimum 3 years of experience in Acute or Managed Care (Medicaid/Medicare populations preferred)
- 3–5 years of experience in Acute Care/Managed Care settings
- Telephonic case management experience
Preferred Qualifications:
- Bilingual in English/Spanish
- Case Management Certification (CCM or similar)
