Are you passionate about providing compassionate, patient-centered care through behavioral health services? Hart Family Medical Associates is looking for a skilled and empathetic Medical Social Worker to join our outpatient care team.
π§ About the RoleAs a Medical Social Worker, you'll support individuals, families, and couples of all ages. You'll conduct assessments, provide therapy, help patients navigate life's challenges, and collaborate closely with our medical providers to coordinate holistic treatment plans.

π What You'll DoPerform comprehensive social work assessments and treatment planning
Deliver individual, family, and group therapy
Maintain timely clinical documentation
Support patients during crises-walk-in or by phone
Partner with providers, insurers, and external agencies
Attend team meetings and contribute to program development
Help shape policies that improve care delivery
Commit to ongoing professional growth
MSW from an accredited Social Work program
Licensed in Michigan (LMSW or equivalent)
At least 2 years' clinical experience
ACSW certification is a plus
Strong communication and collaboration skills
A calm, confident approach in high-stress situations
