The Social Work Case Manager plays a key role in supporting patient flow by coordinating hospital discharges and helping patients and families address factors that impact health outcomes and the ability to age in place. This includes providing emotional support, coordinating community resources, and offering practical assistance.
The Social Worker uses strong analytical, assessment, and interpersonal skills to assist patients and families in navigating complex and challenging situations, ensuring safe and appropriate transitions of care. This role collaborates closely with interdisciplinary team members and, when necessary, engages with external agencies such as Child Protective Services (CPS) and Adult Protective Services (APS).
What is required...
- Master of Social Work degree from an accredited school
- Experience in hospital/acute care or equivalent social service agencies, with a focus on medical social work functions
- BLS Certification (must obtain within 90-days of hire)
What is preferred...
- Hospital inpatient case management experience
- Understanding of patient flow operations
Wage: Wage range will be based on years' of applicable experience. Wage range starts at $31.66/hour and goes up to $47.99/hour
FTE: 1.0 = 40 hours/week
