Work Schedule:

Full-time position with regular business hours, including weekdays, with occasional evening and weekend shifts based on patient needs and assigned caseloads.

 

Responsibilities:

• Conduct psychosocial assessments through interviews, observations, and review of medical records to evaluate patients' emotional, social, and environmental factors impacting their recovery.
• Provide counseling and support to patients and their families, addressing adjustment issues and emotional well-being.
• Collaborate with the rehabilitation team to develop individualized treatment plans, considering patients' psychosocial needs.
• Assist patients in accessing community resources, such as support groups, vocational rehabilitation, or housing assistance.
• Provide support and education to patients and families regarding community resources and post-rehabilitation services.
• Participate in discharge planning and facilitate transitions to home or other care settings.

 

Requirements:

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Experience in a rehabilitation or medical setting is preferred.
• Strong understanding of adjustment issues, grief, and coping mechanisms.
• Excellent communication and interpersonal skills.

 

Salary:

$75,000-100,000 plus bonus
