Full-time/Part-time Social Worker Needed for Community Center in Lexington, NC

Work Hours:
- Monday to Friday: 8:00 AM to 4:30 PM (Full-time schedule)
- Occasional evening or weekend hours may be required based on client needs and availability.
- Flexibility in scheduling is necessary to accommodate visits and ensure continuous care.
- On-call availability may be required to address urgent situations or provide support during off-hours.
- Part-time schedule is based on needs and work hours are negotiable.
Pay:
$60,000 to $100,000 per year

Job Responsibilities:
- Conduct thorough assessments of seniors' psychosocial needs, considering emotional, mental, and social factors.
- Collaborate with clients and their families to develop personalized care plans addressing their specific goals and preferences.
- Offer counseling and emotional support to seniors and their families, helping them navigate life transitions and challenges.
- Advocate for clients' rights and access to community resources, ensuring they receive appropriate support and services.
- Work alongside interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate and enhance care.
- Conduct regular visits to monitor clients' well-being, assess their living conditions, and address any concerns or safety issues.
- Facilitate communication and resolve conflicts among family members involved in the senior client's care.
- Educate clients and their families about available community resources, support groups, and programs to enhance their social engagement and overall quality of life.
- Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and relevant interactions.
- Stay informed about current social work practices, policies, and regulations, ensuring compliance with ethical standards.
Qualifications:
● Master's degree in Social Work from an accredited institution.
● Current and valid state licensure as a Social Worker (LCSW)
● Minimum of two years of experience in a social work role, preferably in a community center or geriatric setting.
● Thorough knowledge of social work principles, aging-related issues, and available community resources.
● Excellent assessment, counseling, and problem-solving skills.
● Ability to empathize, show compassion, and establish rapport with seniors and their families.
● Strong communication and interpersonal skills, enabling effective collaboration with interdisciplinary teams.
● Proficiency in using electronic health record (EHR) systems and computer applications.
