Associate Clinical Social Worker

Associate Clinical Social Worker | San Leandro, CA

Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m.

Salary Range: $66,000 – $80,000, depending on qualifications

Core Duties

1. Provide care coordination across medical and behavioral settings
2. Conduct assessments and support individualized treatment goals
3. Link clients to essential community resources
4. Follow up consistently with clients on progress
5. Collaborate with healthcare and mental health providers

Qualifications

1. Active ASW/ACSW license in California
2. At least one year of clinical or social service experience
3. Strong understanding of local systems of care
4. Clear communication and organizational abilities

Preferred

1. Prior training in integrated healthcare approaches
2. Bilingual abilities, especially Spanish
3. Personal vehicle for field visits
