Key Responsibilities

• Psychosocial Assessment: Conduct thorough psychosocial assessments using evidence-based methodologies that integrate an understanding of human behavior and psychosocial factors influencing treatment outcomes.
• Clinical Interventions: Provide supportive counseling and interventions tailored to patients and families. Facilitate family conferences, especially in navigating complex healthcare decisions such as hospice and advance care planning.
• Triage and Case Management: Efficiently manage your caseload by prioritizing referrals and cases, identifying high-risk situations, and collaborating with Case Managers on discharge planning to promote seamless transitions in care.
• Compliance with Standards: Ensure that all clinical assessments and interventions comply with legal and ethical standards, incorporating knowledge of relevant issues such as potential abuse reporting and patient rights.
• Documentation: Complete all required documentation in accordance with organizational policies, including interdisciplinary plans, social service progress notes, and educational forms.
• In-Service Training: Conduct training sessions for hospital staff on psychosocial challenges and care strategies to raise awareness and enhance service delivery.
• Quality Improvement: Participate actively in quality improvement initiatives by monitoring and analyzing indicators related to patient care outcomes.
• Community Engagement: Involvement in Social Work Month activities, cancer-specific support groups, and educational programs designed for patients, families, and staff.
• Other Duties: Performs additional tasks as assigned that support the team's mission and goals.

Patient Population / Client Focus

The LCSW will primarily work with diverse populations within the community health clinic setting, including individuals facing significant health challenges, familial issues, mental health concerns, and social determinants affecting their wellbeing. Special emphasis is placed on cancer patients and their families, requiring a nuanced understanding of the emotional and psychosocial impacts of diagnosis and treatment.

Qualifications

• A Master’s degree in Social Work from an accredited graduate school of social work is required.
• A minimum of two years of experience in a medical setting is essential, with at least one year focused in a cancer care environment.
• Effective verbal and written communication skills in English, with a proven ability to convey complex information clearly.
• Strong customer service ability and a commitment to patient-centered care.
• Excellent analytical, problem-solving, planning, and evaluation skills are a must.
• The capacity to work autonomously while being an integrated member of a healthcare team.

Preferred Experience

• Experience in facilitating support groups or educational workshops tailored for patients and families.
• Demonstrated competency in electronic health record systems and care coordination tools.

Compensation and Benefits

The annual salary for this role ranges from $120,000 to $130,000, reflecting the candidate's experience and qualifications.
