Key Responsibilities

• Conduct thorough psychosocial assessments through various methods including interviews, patient observations, and analyzing medical records to gauge emotional and social factors impacting students' academic and personal lives.
• Design and implement tailored treatment plans in collaboration with the rehabilitation team, integrating psychosocial strategies aimed at enhancing students' emotional resilience and disruption management.
• Provide individual and family counseling to support emotional adjustment, address anxiety, grief, and develop effective coping mechanisms pertinent to students' unique challenges.
• Connect families with community resources such as support groups, vocational rehabilitation programs, and housing assistance, ensuring students can access necessary services that facilitate their developmental needs.
• Engage in proactive discharge planning for students transitioning out of rehabilitation programs, equipping them and their families with tools and resources to maintain progress independently.

Patient Population / Client Focus

In this role, you will primarily support school-aged children and adolescents dealing with a range of psychosocial issues, including those recovering from physical health challenges, mental health disorders, and social adjustment hurdles. Our mission is to empower students by understanding their individual backgrounds and emotional landscapes, allowing you to address both immediate and long-term needs effectively.

Qualifications

• A Master's degree in Social Work from an accredited program is required.
• Possession of a valid and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of adjustment issues, grief responses, and clinically relevant coping mechanisms.
• Demonstrated ability to communicate effectively and empathetically with diverse populations, including students, families, and team members.

Preferred Experience

• Experience in rehabilitation or medical settings is preferred, particularly involving school-aged populations.
• Familiarity with community resources and support systems available to families in crisis.
• Previous involvement in interdisciplinary collaboration within a healthcare or education-related context.

Compensation and Benefits
