Work Environment

This position is fully remote, allowing you to engage with clients from the comfort of your own home while still fostering meaningful connections through virtual platforms. You will collaborate with educational professionals, families, and interdisciplinary teams, participating in meetings and discussions online. With a standard work schedule of Monday to Friday, 8:00 AM to 4:30 PM, you will have the flexibility to accommodate client appointments and support needs during designated hours.

Key Responsibilities

• Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, incorporating emotional, mental, and social aspects into their care.
• Create and implement personalized care plans in collaboration with clients and their families, ensuring that interventions align with their specific goals and preferences.
• Provide ongoing counseling and emotional support, assisting clients as they navigate transitions associated with aging, such as health changes or loss of independence.
• Advocate for clients' rights and access to community resources, facilitating connections to necessary services like healthcare, housing, and social activities.
• Cooperate closely with a diverse team, including healthcare providers and educational staff, to ensure holistic care that addresses all aspects of clients’ lives.
• Conduct regular check-ins with clients to monitor their progress and adjust care plans as needed based on evolving needs and circumstances.
• Educate clients and their caregivers about relevant community resources, available support networks, and strategies to enhance social engagement and quality of life.
• Maintain thorough and accurate documentation of client interactions, assessments, care plans, and progress, ensuring readiness for audits and organizational reviews.
• Stay updated on industry best practices, social work policies, and ethical standards to ensure compliance and provide the highest level of service.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A current and valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of relevant experience in a social work capacity, ideally in geriatric or community-centered environments.
• A solid understanding of social work best practices, particularly concerning elder care issues and resources available to seniors.
• Exceptional assessment, counseling, and problem-solving abilities, with a strong capacity for empathy and communication.
• Experience in building relationships with seniors and their families, demonstrating compassion and understanding.
• Proficiency in using electronic health records (EHR) and other digital tools to document interactions and assess client needs.

Preferred Experience

• Experience working within educational settings is an advantage, as this role collaborates closely with school staff to better support seniors.
• Familiarity with community resources specific to Houston, Texas, and surrounding areas, aiding in effective advocacy for clients.
