Key Responsibilities

• Conduct thorough assessments to identify the social, emotional, and environmental challenges faced by clients, utilizing standardized assessment tools and client interviews.
• Develop and implement individualized service plans that outline specific goals, interventions, and methods for tracking progress, while maintaining client-centered care throughout the process.
• Provide empathetic counseling to individuals and families dealing with crises, assisting them in navigating their challenges and reinforcing their strengths.
• Collaborate closely with clients to design personalized service plans, connecting them to critical community resources and services that can address their unique situations.
• Engage in crisis intervention, performing risk assessments, and developing safety plans as needed for clients in acute distress.
• Work with community agencies to coordinate care and advocate for clients by identifying barriers to access and facilitating connections to needed resources.
• Participate in community outreach initiatives to raise awareness of available services and encourage community engagement.
• Attend case conferences, team meetings, and staff training to enhance communication and exchange knowledge with colleagues.

Patient Population / Client Focus

In this role, you will primarily support individuals and families from diverse backgrounds who may be experiencing significant socio-economic challenges. Your focus will include populations affected by homelessness, substance dependency, domestic violence, and mental health issues. By addressing these critical areas, you will help empower clients to reclaim their agency and improve their overall well-being.

Qualifications

• A Master’s degree in Social Work (MSW) from an accredited program is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory prior to employment.
• Strong knowledge of trauma-informed care practices and understanding of social determinants affecting health and wellness.
• Demonstrated ability to communicate effectively with a diverse clientele, showcasing cultural competence and sensitivity to varied backgrounds.

Preferred Experience

• Prior experience within a social service agency or a similar setting, particularly in roles focused on case management and counseling, is highly preferred.
• Familiarity with community resources available within San Jose and the surrounding areas to aid client referrals.
• Experience with electronic health record (EHR) systems or case management software to manage client documentation and track progress.

Compensation and Benefits
