Key Responsibilities

• Engage directly with assigned clients, assessing their needs and developing personalized care plans to address symptoms of major mental illness and emotional disorders.
• Work collaboratively with a multidisciplinary team and contribute to shared goals, enhancing communication and fostering a supportive atmosphere for both clients and colleagues.
• Conduct evidence-based screenings and assessments, utilizing appropriate methodologies to ensure accurate diagnosis and effective treatment planning.
• Create and implement integrated care plans that ensure continuity of services, including facilitating communication between clients, family members, and healthcare providers.
• Offer a range of brief treatment, prevention, and recovery services, adapting your approach to individual client needs and cultural contexts.
• Maintain a caseload effectively, providing case management including individual, group, and family therapy interventions.
• Participate in crisis intervention when necessary, employing skills to manage acute situations and direct clients to appropriate resources.
• Ensure compliance with professional ethical guidelines and contribute to maintaining a safe and supportive therapeutic environment.
• Submit timely reports and accurately document care and treatment interventions to support quality improvement efforts.
• Participate in agency committees aimed at enhancing service delivery and exploring innovative solutions for challenges faced by the organization.

Patient Population / Client Focus

Your primary focus will be on clients dealing with the emotional and psychological strain of chronic illnesses in a hospice setting. You will interact and provide support to clients from diverse backgrounds, ensuring that cultural and social nuances are respected and incorporated into care provisions. Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Must hold a valid LCSW license to practice in New York State.
• Excellent communication skills, both verbal and written, allowing for effective collaboration with team members and clear communication with clients.
• Strong organizational and planning skills, with the ability to manage multiple priorities in a remote setting.
• Demonstrated competency in addressing various age-related or specialty mental health issues impacting the populations served.

Preferred Experience

• A minimum of 2 years of experience as a clinical social worker, ideally with a focus on mental health, crisis intervention, or hospice care.
• Proven experience in developing and implementing care strategies for individuals experiencing severe emotional or psychological distress.

Compensation and Benefits

This position offers a competitive salary ranging from USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
