Key Responsibilities

• Act as a crucial liaison for families and outpatient providers, ensuring an integrated approach to care.
• Engage with insurance companies for pre-certification and utilization management, advocating for appropriate coverage.
• Contribute to discharge and aftercare planning, working in collaboration with multidisciplinary teams to address ongoing needs of clients.
• Provide therapeutic meal support, guiding clients in developing a healthy relationship with food while modeling appropriate behaviors.
• Deliver empathetic, affirming, and culturally sensitive care that considers clients' diverse intersecting identities.

Patient Population / Client Focus

This position primarily supports older adults facing terminal illnesses and their families. You will work closely with clients who are navigating profound emotional complexities, guiding them through the end-of-life process, and addressing their mental health needs. Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of New Jersey is mandatory.

• Excellent communication and interpersonal abilities to build trust and rapport with clients and families.

Compensation and Benefits

The compensation for this role ranges from $90,000 to $105,000 per year, commensurate with experience and qualifications. Additional benefits include:
