Acting as a liaison between clients, their families, and outpatient providers to ensure seamless communication and care coordination.
Engaging with insurance companies for pre-certification and utilization management to advocate for client needs and ensure appropriate coverage for essential services.
Participating actively in discharge planning and aftercare to facilitate a smooth transition for clients re-entering the community or engaging with continuing care options.
Providing therapeutic meal support, emphasizing the development of a healthy relationship with food through modeling and guidance.
Delivering empathetic and affirming care tailored to each client’s unique circumstances, respecting their intersecting identities and experiences.
Your role primarily focuses on serving a diverse client population with a variety of mental health needs, including adults and older adults. By understanding and addressing these complexities, you will help clients embark on their journey to healing and empowerment.
A Master’s degree in Social Work from an accredited institution is required.
A valid LICSW license in the state of Washington is essential for this position.
Demonstrated experience working with older adults and an understanding of the mental health challenges they face.
Strong clinical and diagnostic skills are crucial for effective patient assessment and intervention.
Exceptional communication and interpersonal abilities to foster trust and rapport with clients and families.
Must possess the capability to work both independently and collaboratively within a team-oriented setting.
Experience in community mental health settings or addiction services is beneficial. Additionally, familiarity with electronic health record systems and telehealth practices will enhance your efficiency and effectiveness in supporting our clients.
