Work Environment

Our team operates in a remote setting, allowing for flexibility in work hours while maintaining a structured schedule from Monday to Friday, 9:00 AM to 5:00 PM. You will leverage telehealth technologies to connect with clients effectively and provide ongoing support.

Key Responsibilities

In this role, your duties will include:

• Conducting thorough psychosocial assessments of clients to identify their emotional, social, and environmental needs, as well as their strengths and challenges.
• Delivering individualized counseling and mental health education, utilizing evidence-based therapeutic modalities tailored to the unique experiences of each client.
• Developing and implementing personalized care plans in collaboration with clients and an interdisciplinary healthcare team, focusing on strategies for behavior modification, coping mechanisms, and lifestyle enhancements.
• Assisting clients in setting realistic and achievable goals to help them manage their chronic conditions and mental health issues more effectively, both in synchronous and asynchronous formats.
• Executing regular outreach efforts to clients based on their preferences and needs to promote engagement and facilitate positive changes in their behavior.
• Accurately documenting all client interactions, progress notes, and care plans in our electronic health record (EHR) system, ensuring compliance with necessary regulations and best practices.

Qualifications

• A Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of three years of experience working in a mental health or outpatient setting.
• Strong understanding of evidence-based therapeutic modalities and current best practices in mental health treatment.
• Exceptional interpersonal, communication, and assessment skills to effectively connect with clients and their families.

Preferred Experience

• Experience utilizing teletherapy platforms or remote counseling techniques to engage with clients.
• Familiarity with integrated healthcare environments and collaborative care models.
• Background in community outreach initiatives, emphasizing strategies that promote client engagement and empowerment.

Compensation and Benefits

This position offers a competitive salary range of USD 90,000 – 100,000 annually, complemented by a comprehensive benefits package that includes:
