Work Environment

This role offers the flexibility of a remote work setting, providing a supportive virtual environment where staff can connect and collaborate. The full-time schedule is structured from Monday to Friday, 9:00 AM to 5:00 PM, allowing for work-life balance while delivering essential mental health services. Key Responsibilities

• Act as a vital liaison between clients, their families, and outpatient providers to facilitate seamless communication and care coordination.
• Engage with insurance companies to manage pre-certification processes and utilization management to ensure clients receive timely care.
• Participate actively in discharge planning and aftercare processes, collaborating with a multidisciplinary team to promote continuity of care.
• Provide therapeutic meal support, aiming to model and instill a healthy relationship with food among clients.
• Deliver safe, empathetic, and affirming care tailored to each client’s unique circumstances, taking into account their intersecting identities.

Patient Population / Client Focus

In this role, you will primarily work with a diverse array of clients, including older adults, people struggling with mental health disorders, and those seeking support for relationship challenges. Understanding the multifaceted issues these individuals may face will be crucial to providing effective, person-centered care that addresses their specific needs and situations.

Qualifications

• A Master’s degree in social work from an accredited institution.
• Possession of a valid LCSW license in the state of Maryland is required.
• A minimum of two years of relevant experience working with older adults or diverse populations in a mental health setting.
• Strong clinical and diagnostic skills, along with the ability to develop effective treatment plans.
• Excellent verbal and written communication skills, fostering positive interactions with clients and colleagues alike.
• Ability to work both independently and collaboratively within a multidisciplinary team environment.

Preferred Experience

• Experience in community health settings, specifically within a clinical outpatient environment.
• Familiarity with electronic health record (EHR) systems and telehealth practices.
• Demonstrated ability to work with clients from various cultural backgrounds and socioeconomic statuses.

Compensation and Benefits

The salary range for this full-time LCSW position is $90,000 to $105,000 annually, commensurate with experience.
