Experity Care Consultants is seeking a Full-Time Licensed Clinical Social Worker (LCSW) to join our dedicated team focused on providing exceptional hospice care. This remote position allows you to support patients and their families through some of life’s most challenging moments, offering not only clinical support but also compassion and understanding during their journey.

This position is entirely remote, providing flexibility while still ensuring that you can engage meaningfully with patients and their families. As part of a hospice center, you will work closely with a team of dedicated professionals, including physicians, nurses, and other healthcare workers, to ensure tailored and coordinated care for those facing end-of-life challenges. Our team is designed to foster collaboration and communication, even from a distance, ensuring that every patient receives the highest standard of care.
Your primary focus will be supporting individuals receiving hospice care, where you’ll assist in addressing both psychological and emotional needs. This may include working with patients and families who are navigating end-of-life issues, grief, and complicated emotional reactions to declining health circumstances, always aiming to enhance their quality of life. You will play an integral role in ensuring that families receive the support they need as they prepare for significant transitions.
