Key Responsibilities

• Lead psychoeducational and process-oriented therapy groups designed to address diverse mental health challenges, enhancing clients’ understanding of their conditions.
• Provide individual therapy sessions, creating personalized treatment plans that align with best practices and evidence-based approaches.
• Conduct thorough clinical assessments to identify patient needs, symptoms, and support systems, ensuring comprehensive care plans are developed expediently.
• Initiate discharge planning on the patient's day of admission, involving them in discussions about coping strategies, transition protocols, and resource identification.
• Facilitate effective discharge sessions that reinforce patients' understanding of their care plans, connecting them with necessary resources and solidifying their transition to post-treatment support.
• Ensure continuous communication with referral sources and outpatient providers during the patient's journey, including admission, midpoint, and discharge phases, to promote continuity of care.
• Manage patient transfers efficiently when higher levels of care are required, advocating for their needs during crucial decisions.

Qualifications

• Master’s degree in Social Work from an accredited institution.

• Proven ability to maintain healthy boundaries with clients while demonstrating empathy and understanding.
• Strong organizational skills, with the capability to complete and submit necessary documentation of services in a timely manner to ensure compliance with regulatory standards.

Compensation and Benefits
