Key Responsibilities

• Engage and establish rapport with clients who are facing symptoms of major mental illness or severe emotional disorders.
• Conduct comprehensive assessments to develop integrated care plans tailored to the unique needs of hospice clients.
• Provide brief, focused treatment, counseling, and recovery services, addressing immediate concerns and emotional needs.
• Participate actively in team meetings and collaborate with other healthcare providers to ensure continuity of care.
• When necessary, conduct further detailed assessments and ensure accurate documentation of findings.
• Create and maintain a safe environment by identifying unsafe situations and implementing appropriate interventions.
• Assist in crisis intervention, providing immediate support and assistance during high-stress situations for clients and families.
• Monitor and manage a caseload of clients, ensuring timely follow-ups on treatment goals and outcomes.
• Engage in professional development activities to maintain and enhance clinical skills, including staying updated on best practices in social work and hospice care.
• Ensure compliance with ethical guidelines and organizational policies while delivering exceptional care to our clients.

Patient Population / Client Focus

Your primary focus will be on clients receiving hospice care, many of whom may experience complex emotional states stemming from terminal illness. Your role will encompass providing empathetic support during a vulnerable time in their lives. You will work with diverse populations, adapting your approach to reflect the various cultural, belief, and value systems that influence how clients and families navigate illness and grief.

Qualifications

• Must possess a current and valid LCSW license in the state of Tennessee.
• A Bachelor’s degree in Social Work or a related field is required; further qualifications are advantageous.
• At least 2 years of clinical experience, particularly in hospice, palliative care, or mental health settings.
• Strong interpersonal skills, with the ability to foster trusting and respectful relationships with clients and their families.
• Ability to work independently and collaboratively, demonstrating a commitment to team success and client outcomes.

Preferred Experience

• Prior experience working in hospice or end-of-life care environments is highly desirable.
• Familiarity with common emotional disorders presented by clients in hospice settings.
• Experience in crisis intervention and developing safety plans.
• Proficiency in creating and implementing care plans that are holistic and client-centered.
• Knowledge of community resources available for clients and families in need of additional support.

Compensation and Benefits

This position offers a competitive salary range of USD 70,000–85,000 annually, commensurate with experience and qualifications.
