Work Environment

Key Responsibilities

• Conduct comprehensive psychosocial assessments tailored to clients’ mental health concerns, strengths, and available resources.
• Deliver individual, family, and group therapy sessions utilizing evidence-based approaches, including cognitive-behavioral therapy and solution-focused therapy.
• Develop and implement individualized treatment plans, articulating clear goals and therapeutic interventions relevant to each client's unique circumstances.
• Collaborate closely with healthcare professionals, including psychiatrists and psychologists, to coordinate all facets of client care effectively.
• Provide crisis intervention, risk assessment, and safety planning at critical moments for clients and their families.
• Facilitate psychoeducation classes and therapeutic support groups to foster a sense of community and understanding.
• Thoroughly document all client interactions and maintain accurate records that comply with ethical and legal standards.

Patient Population / Client Focus

Your primary focus will be on clients within a hospice setting, including individuals facing terminal illnesses, their families, and caregivers. You will address complex emotional and psychological needs, working to alleviate anxiety, depression, and grief as clients navigate their experiences and transitions toward end-of-life care.

Qualifications

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a mental health or outpatient setting, with a preference for experience in hospice or palliative care.
• Demonstrated expertise in a variety of evidence-based therapeutic modalities, including trauma-informed care.
• Exceptional interpersonal and communication skills, alongside strong assessment capabilities.

Preferred Experience

• Experience working in hospice care or with terminally ill patients and their families.
• Proficiency in using telehealth platforms and electronic health records (EHR) systems.

Compensation and Benefits
