Key Responsibilities

• Conduct comprehensive assessments of clients’ mental health and social needs, ensuring that interventions are customized to each individual.
• Develop and implement personalized treatment plans that incorporate evidence-based solutions, focusing on therapeutic modalities to enhance patient well-being.
• Maintain detailed and accurate clinical records, ensuring compliance with HIPAA regulations and confidentiality standards.
• Collaborate effectively with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, for holistic patient care.
• Provide ongoing education and support to patients and their families about coping strategies, treatment options, and available resources.
• Stay abreast of the latest research and developments in mental health and hospice care to ensure the highest standard of practice.

Patient Population / Client Focus

As a Licensed Clinical Social Worker in our Hospice Center, you will primarily support patients facing terminal illnesses, providing them with emotional support and therapeutic interventions aimed at alleviating anxiety, depression, and trauma associated with their illnesses. You will work closely with both patients and their families, helping them navigate the emotional complexities of end-of-life care and fostering a sense of peace and dignity in their remaining time.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW-C license in the state of Maryland.
• Minimum of 2 years of clinical experience in a mental health setting or hospice care.
• Strong proficiency in solution-based psychotherapy interventions, particularly with trauma and end-of-life issues.
• Excellent communication and interpersonal skills, fostering rapport and trust with patients and families.

Preferred Experience

• Experience working in hospice or palliative care settings.
• Skills in using electronic health records (EHR) systems to maintain accurate documentation.
• Familiarity with holistic approaches and interdisciplinary teamwork in a healthcare environment.

Compensation and Benefits
