Key Responsibilities

• Conduct thorough assessments of clients’ social, emotional, and environmental challenges to create a tailored and effective service plan.
• Develop and implement personalized intervention strategies encompassing individual and family counseling, achieving both short-term and long-term goals.
• Provide crisis intervention services, which include risk assessments and planning for client safety during critical situations.
• Serve as a liaison with community agencies and organizations, ensuring clients are connected with relevant resources and services to address their unique needs.
• Participate actively in case conferences and interdisciplinary team meetings, sharing insights and fostering collaboration among various support providers.
• Engage in community outreach initiatives aimed at raising awareness of social issues and promoting the agency’s services.

Patient Population / Client Focus

You will be directly supporting clients facing diverse challenges, including homelessness, domestic violence, substance abuse, and mental health crises. Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Candidates must hold an active state licensure as a Licensed Clinical Social Worker (LCSW).

• Excellent communication skills, with a demonstrated ability to interact effectively with individuals from diverse backgrounds.
