Licensed Clinical Social Worker - Remote Hospice Care Specialist

Aviva Healthcare, LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote hospice care team in Corpus Christi, Texas. Work Environment

As a remote position within our Hospice Center, you will have the flexibility to manage your schedule while ensuring that patients receive the support they need. While the role can often be performed from home, you may also attend occasional team meetings and client visits as required based on individual needs. Key Responsibilities

• Conduct thorough psychosocial assessments to understand clients' mental health concerns, strengths, and available resources, guiding tailored interventions.
• Deliver individual, family, and group therapy sessions utilizing proven evidence-based treatment methodologies such as cognitive-behavioral therapy (CBT) and solution-focused therapy.
• Develop and implement comprehensive treatment plans, incorporating specific goals and therapeutic interventions based on individual client needs.
• Collaborate regularly with psychiatrists, psychologists, and other healthcare professionals to ensure cohesive, client-centered care across disciplines.
• Provide crisis intervention, perform risk assessments, and engage in safety planning, especially for clients in acute distress.
• Maintain accurate documentation in accordance with ethical and legal guidelines to ensure continuity of care and adherence to regulatory standards.
• Facilitate group therapy sessions, education classes, and support groups aimed at enriching client understanding and coping skills.

Patient Population / Client Focus

Your primary clientele will encompass a diverse population facing chronic or terminal illnesses, requiring mental health support during pivotal moments of their life journey. You will champion the psychosocial well-being of clients as they navigate the complexities of grief, loss, and illness, and empower families in the caregiving process. Your role is pivotal in fostering resilience and healing, ensuring that clients feel valued and understood.

Qualifications

• A Master’s degree in Social Work from an accredited program is essential.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice.
• A minimum of two years of experience in a mental health or outpatient setting, particularly with hospice or palliative care experience preferred.
• Demonstrated knowledge of evidence-based therapeutic modalities and the ability to apply them effectively.
• Strong interpersonal, communication, and assessment skills, with the capacity to build trusting relationships with clients and families.

Preferred Experience

• Experience working within hospice, palliative care, or similar settings.
• Familiarity with electronic health record (EHR) systems and teletherapy platforms to facilitate remote client engagement.
• Specialized training in trauma-informed care or mindfulness-based approaches is a plus.

Compensation and Benefits
