Key Responsibilities

• Conduct thorough clinical assessments through telehealth platforms, identifying the unique needs of patients under hospice care.

• Document clinical records in a secure electronic health record (EHR) system, ensuring compliance with regulatory standards and facilitating care continuity.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
