Stream Mindful Medication, Inc. is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote capacity. As an LCSW with us, you will leverage your clinical expertise to support a diverse patient population in a hospice center setting, focusing on delivering high-quality mental health care while ensuring emotional and psychological support at critical moments in their lives.

This role operates in a fully remote work setting, offering flexibility to accommodate the personal schedules of our staff. You will collaborate virtually with a multidisciplinary team of healthcare professionals, including psychologists, primary care providers, and other mental health specialists. While the position is primarily remote, you may be required to engage in some evening and weekend hours to best support our patients and their families.
In this role, you will predominantly work with patients in hospice care, providing essential emotional support during end-of-life transitions. Your expertise will be invaluable in navigating common emotional challenges such as grief, anxiety, and depression that families face. You will serve as an integral resource for patients and their families, offering a comforting presence and fostering resilience through the therapeutic process.
While not required, prior experience working in hospice, palliative care, or with terminally ill patients will be viewed favorably. Familiarity with interdisciplinary teamwork, psychological therapies, and holistic health practices is also ideal.
