Key Responsibilities

• Acting as a liaison between families and outpatient providers to coordinate care effectively, ensuring that the patient’s needs are prioritized and addressed.
• Collaborating with insurance companies for pre-certification and utilization management, helping to navigate the complexities of patient care coverage.
• Participating actively in discharge and aftercare planning, guiding patients and families through the transition phases with understanding and support.
• Providing therapeutic meal support to clients, incorporating strategies that model a healthy relationship with food while addressing the psychological aspects of nutrition.
• Delivering safe, empathetic, and affirming care tailored to each client’s unique circumstances, taking into account their intersecting identities, cultural backgrounds, and personal histories.

Patient Population / Client Focus

In this role, you will primarily work with older adults experiencing complex emotional and psychological challenges associated with hospice care. Your expertise will enable you to provide nuanced, person-centered support that acknowledges the unique experiences of each client and their families. By fostering a trusting relationship, you will help clients cope with grief, loss, and transitions, facilitating improved emotional well-being during sensitive times.

Qualifications

• Master's degree in Social Work from an accredited institution is required to ensure a strong foundation in clinical practice.
• A valid LCSW license in the state of Colorado is mandatory to practice within the hospice care environment.
• Demonstrated experience working with older adults and understanding the particular challenges they face in hospice settings.
• Strong clinical and diagnostic skills that allow for comprehensive assessments and informed treatment planning.
• Excellent communication and interpersonal skills to effectively engage with patients, families, and a diverse range of healthcare providers.

Compensation and Benefits
