Key Responsibilities

• Conduct thorough psychosocial assessments that identify and evaluate the emotional, social, and financial needs of students and their families.
• Deliver both individual and group counseling sessions focused on topics such as coping strategies, grief, and adjustment challenges.
• Engage regularly with teachers and school administrators to facilitate a supportive environment for students' mental health.
• Develop discharge plans tailored to each student, coordinating with various community resources to ensure continuity of care post-discharge.
• Participate in case management meetings, continuously collaborating with the interdisciplinary team to optimize patient outcomes.
• Maintain accurate and timely documentation of all counseling sessions and interactions in compliance with clinic protocols and legal standards.
• Provide crisis intervention services and offer immediate emotional support to students and their families in distress.
• Develop and deliver educational workshops aimed at empowering students and families with knowledge about mental health resources and coping mechanisms.
• Actively participate in quality improvement initiatives and contribute insights on social work-related matters in committee meetings.

Patient Population / Client Focus

The primary focus of this role is on children and adolescents attending local schools within the community served by Los Barrios Community Health Clinic. You will address a broad spectrum of psychosocial issues, collaborating with families to navigate challenges related to mental health, academic stressors, and overall well-being. Providing culturally competent care that respects the diverse backgrounds of our students is essential in achieving successful outcomes.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
• Strong foundational knowledge of psychiatric conditions, familial systems, and therapeutic interventions.
• Proficient in maintaining comprehensive documentation and adhering to legal and ethical standards in social work practice.

Preferred Experience

• A minimum of two years of experience in a behavioral health setting, preferably involving school-based services or community mental health.
• Experience in crisis intervention and trauma-informed care approaches.
• Familiarity with educational systems and the specific mental health challenges faced by students.

Compensation and Benefits
