Work Environment

Key Responsibilities

• Conduct comprehensive intake assessments and psychosocial evaluations to understand the needs of patients and their families.
• Develop tailored treatment plans that address the unique challenges faced by patients, presenting them to the Clinical Development and Review Process (CDRP) Treatment Team for collaboration and approval.
• Participate actively in clinical case conferences to discuss patient care strategies and making necessary adjustments based on feedback and overall patient progress.
• Deliver educational lectures that center around chemical dependency, aimed at enhancing patients' coping mechanisms and fostering understanding of addiction.
• Provide individual and group counseling services focused on chemical dependency, supporting patients through their recovery journey.
• Offer specialized therapy sessions including adolescent therapy, family therapy, and multi-family therapy as needed, ensuring that all family dynamics are addressed effectively.
• Coordinate case management services, serving as a resource guide and helping clients navigate community resources and referrals.

Patient Population / Client Focus

The ideal candidate will engage with a diverse array of clients, primarily focusing on patients in hospice care who are dealing with life-limiting illnesses. You will work closely with patients and their families, providing essential support, education, and counseling as they navigate palliative care options. Your role will be crucial in ensuring the emotional and psychological needs of clients are met, creating a holistic approach to care.

Qualifications

• A Master's Degree in Social Work (MSW) or a closely related field is required.
• Current Licensure as an Independent Clinical Social Worker (LICSW) in Washington DC.
• A minimum of two (2) years of post-graduate experience in a relevant clinical setting, particularly in addiction medicine therapy, within the last five (5) years is essential.

Preferred Experience

Experience in hospice care or working with terminally ill patients will be prioritized. Familiarity with interdisciplinary team dynamics and a strong background in family therapy will enhance your ability to succeed in this role. Your past work with community resources will further empower you to make necessary referrals and enhance patient outcomes.
