Licensed Clinical Social Worker for School-Based Services

Position Overview

Berks Community Health Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in Lebanon, Pennsylvania. This full-time, remote position focuses on providing essential behavioral health services within a school setting. The ideal candidate will play a critical role in addressing the mental health needs of students and their families, ensuring they have access to the resources and support necessary to thrive academically and emotionally.

Work Environment

This position offers a remote work setting, allowing for flexibility while serving students in local schools. The successful candidate will collaborate with behavioral health and primary care providers to create an integrated care approach, ensuring that students receive the support they need in both their educational and personal lives. As a member of a professional team, the LCSW will engage in regular meetings and collaborative discussions to foster continuous improvement in service delivery.

Key Responsibilities

• Establish rapid rapport and clear communication with students, families, and other team members utilizing active listening and a non-judgmental, jargon-free approach.
• Function effectively as part of a multidisciplinary team that includes behavioral and primary care providers to identify and address mental health concerns.
• Conduct brief, evidence-based screenings for conditions such as depression, anxiety, and substance abuse, and arrange for comprehensive assessments when necessary.
• Create and manage a patient caseload, providing follow-up interventions and ensuring continuity of care.
• Utilize electronic medical record systems to document patient encounters efficiently and adhere to a 48-hour turnaround for documentation.
• Develop and implement integrated care plans, facilitate communication among all parties involved in the patients’ care, and identify needed services.
• Safeguard patient privacy and confidentiality throughout all interactions and documentation.
• Participate actively in individual and group supervision sessions, case conferences, and quality improvement meetings.
• Facilitate referrals for psychiatric evaluations when medications are prescribed by BCHC’s medical providers.
• Return calls promptly to patients, families, and treatment providers to address appointment requests and concerns.
• Identify community resources and support networks that align with the goals and interests of patients.
• Perform additional duties as required to support departmental objectives.

Patient Population / Client Focus

The LCSW will primarily focus on serving students and their families from underserved and diverse backgrounds in Lebanon, Pennsylvania. This role requires cultural competence and an understanding of the unique challenges faced by homeless individuals and marginalized groups. Utilizing trauma-informed care and a social justice framework is essential for effectively meeting the needs of this population.

Qualifications

• A Master’s degree in a related Behavioral Health field.
• Current LCSW licensure is required.
• CPR or BLS certification is mandatory.
• A minimum of three years of experience in the behavioral health field.
• Experience working with culturally diverse populations, including a commitment to understanding the needs of underserved communities.
• Proficiency in the use of electronic health record systems and a familiarity with office management software.
• Bilingual proficiency in English and Spanish is preferred but not required.

Preferred Experience

• Previous experience in a school-based setting is an asset.
• Demonstrated expertise in applying behavioral medicine principles and evidence-based practices for treating mental health conditions in a primary care environment.
• Experience with crisis intervention and management across different levels of patient needs.

Compensation and Benefits
