Key Responsibilities

• Conduct comprehensive assessments of patients’ emotional and mental health needs, ensuring accurate diagnoses and treatment planning.
• Develop tailored treatment plans based on individual assessments that outline the patient's goals and desired outcomes.
• Offer empathetic and compassionate support to patients and their families navigating complex emotional landscapes related to end-of-life care.
• Utilize evidence-based therapeutic methods, including cognitive-behavioral therapy and mindfulness techniques, to facilitate patient progress.
• Maintain accurate documentation of patient interactions, treatment plans, and progress notes in compliance with confidentiality standards.
• Participate in professional development sessions to continually enhance your knowledge of best practices in mental health care.

Patient Population / Client Focus

You will primarily engage with patients in a hospice care setting, providing emotional support during a profoundly challenging time in their lives. Your role is critical in helping patients and their families cope with the emotional weight of end-of-life scenarios, providing them with necessary tools to process their experiences and navigate their feelings.

Qualifications

• A Master’s degree in Social Work (MSW) with an active LCSW license in Oregon is mandatory.
• Minimum of two years of direct clinical experience as a Licensed Clinical Social Worker (LCSW), preferably in a mental health or hospice care setting.
• Proficient in telehealth service delivery, with the technical skills necessary to utilize virtual platforms effectively.
• In-depth understanding of mental health diagnoses, treatment modalities, and ethical practices within a clinical setting.

Preferred Experience

• Experience working with a hospice patient population or familiarity with the specific needs and challenges faced by individuals nearing the end of life.
• Knowledge of best practices for end-of-life counseling and bereavement support.
• Previous engagement with interdisciplinary teams to provide holistic patient-centered care.

Compensation and Benefits
