Licensed Clinical Social Worker (LCSW) - Remote (Hospice Focus)

Position Overview

The Licensed Clinical Social Worker (LCSW) is a pivotal role within our organization, responsible for delivering compassionate mental health care to individuals of all ages, particularly those facing terminal illnesses. Key Responsibilities

• Provide individualized mental health therapy to clients, including individuals, couples, families, and children, tailored to each client's unique needs and backgrounds.
• Conduct comprehensive intake assessments to understand clients' situations, strengths, and challenges, guiding the development of effective treatment plans.
• Develop and implement evidence-based treatment methodologies, facilitating both group therapy and one-on-one sessions to foster growth and healing.
• Coordinate with the Utilization Review Department regarding treatment authorizations and billing processes, ensuring clients receive the appropriate level of care.
• Perform regular case management duties, including tracking clients' progress, reassessing needs, and adjusting treatment plans as required to enhance therapeutic outcomes.
• Engage in outreach efforts to educate and connect community members with necessary resources, improving overall mental health support in the area.
• Document services provided and monthly progress notes promptly, adhering to compliance standards and organizational policies.

Patient Population / Client Focus

Our client base primarily includes individuals and families experiencing complex grief and mental health challenges associated with terminal illnesses. The LCSW will focus on providing a therapeutic atmosphere that supports emotional healing and adaptation, addressing both psychological distress and practical concerns faced by the hospice patient population.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• An active and valid Clinical Social Worker (LCSW) license in California.
• Strong written and verbal communication skills, with the ability to articulate clinical assessments and treatment plans effectively.
• Proven ability to establish and maintain appropriate professional boundaries while developing rapport with clients.
• Timeliness and thoroughness in completing all clinical documentation and submitting necessary reports.

Preferred Experience

• Experience working in hospice, palliative care, or with end-of-life issues is highly preferred.
• Demonstrated effectiveness in handling a diverse client population, addressing a wide array of mental health issues.
• Familiarity with electronic health record systems and documentation practices in a digital workspace.
• Skills in crisis intervention and risk assessment, particularly concerning grief and loss.

Compensation and Benefits
