Key Responsibilities

• Conduct comprehensive mental health assessments, considering how medical conditions impact psychological well-being.
• Design and implement integrated treatment plans that effectively address both mental health and medical issues.
• Facilitate individual and group therapy sessions, utilizing evidence-based therapeutic modalities tailored to the specific needs of each patient.
• Engage in collaborative care with interdisciplinary medical providers to ensure holistic treatment and positive patient outcomes.
• Provide crisis intervention and immediate support during mental health emergencies, ensuring patient safety and well-being.
• Empower patients through education about the interplay between mental health and physical conditions, enhancing their ability to manage their overall health.
• Maintain precise and confidential records in strict accordance with legal and ethical guidelines.
• Actively engage in team meetings and case conferences to discuss patient progress and adjust treatment plans as necessary.
• Stay informed about current research and advancements in integrated care practices to continually enhance service delivery.

Patient Population / Client Focus

Your work will directly impact a diverse range of patients battling both mental health challenges and physical health issues. These may include, but are not limited to, individuals coping with anxiety, depression, PTSD, substance abuse disorders, and chronic illnesses. Qualifications

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current licensure as an LPCC, LCSW, or LMFT in the state of California.
• Demonstrated experience supporting individuals with co-occurring medical and mental health challenges.
• Exceptional interpersonal and communication skills are essential for effective patient engagement and collaboration with colleagues.
• Ability to maintain professional boundaries and uphold ethical standards while respecting patient confidentiality.
• Familiarity with electronic health record systems and the ability to navigate digital platforms for documentation and patient management.
• Strong team collaboration skills that facilitate a positive work environment and enhance patient care.

Preferred Experience

• Three years of relevant experience in a clinical setting focused on integrated mental health care.
• Utilization of evidence-based practices in a therapeutic context, particularly for treating patients with chronic health issues.
• Experience working within community health or outpatient settings.

Compensation and Benefits
