Position Overview

The Trillium Health Center is seeking a passionate and dedicated Licensed Independent Clinical Social Worker (LICSW) to join our team in providing vital support within our Hospice Center. Key Responsibilities

• Provide clinical interventions to clients experiencing major mental illness and severe emotional disorders, utilizing evidence-based practices tailored to individual needs.
• Participate actively as a collaborative team member, contributing to shared goals and program development within our hospice framework.
• Conduct thorough assessments, screenings, and evaluations with a focus on client empowerment, delivering interventions that prioritize client choice.
• Develop and implement integrated care plans that coordinate access to necessary services, ensuring smooth communication among clients, families, and healthcare providers.
• Manage an assigned caseload, offering individual, group, and family therapy sessions that focus on current treatment goals and client-driven outcomes.
• Engage in crisis intervention as needed, addressing urgent situations with professionalism and urgency.
• Monitor and document client progress, including admission criteria, treatment updates, and discharge planning, ensuring timely reporting and compliance with organizational policies.
• Contribute to quality improvement committees, utilizing feedback to enhance service delivery and patient experiences.

Patient Population / Client Focus

As a licensed clinical social worker at Trillium Health Center, you will work primarily with hospice patients and their families, providing psychological support during some of the most challenging times in their lives. Your role will require sensitivity and understanding as you navigate complex emotional and psychological needs, all while respecting the diverse cultural backgrounds and beliefs of clients. This commitment to cultural competence is essential as it directly relates to the effectiveness of care provided.

Qualifications

• A Master's degree in Social Work (MSW) or related field is required for this position.
• Current LICSW licensure in the state of Massachusetts is mandatory.
• A minimum of two years of experience working as a clinician, particularly in mental health or hospice settings, is preferred.
• Strong verbal and written communication skills to facilitate effective interaction with clients, families, and healthcare professionals.
• Proficient organizational and planning skills to manage a diverse caseload efficiently.
• Understanding of age-specific and specialty care needs related to the populations served.

Preferred Experience

• Bilingual proficiency in English and Spanish is highly desirable, as it enhances communication with a broader client demographic.
• Experience with electronic health record (EHR) systems and telehealth platforms, facilitating efficient client monitoring and documentation.

Compensation and Benefits
