Support Wellness Healthcare Services, LLC is seeking a dedicated and professional Licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote capacity. In this role, you will facilitate therapy sessions, lead psychoeducational and process-oriented groups, and conduct comprehensive clinical assessments. Work Environment

The position operates within a remote framework, allowing flexibility for our Licensed Clinical Social Worker. You will be part of a collaborative virtual team that includes therapists, healthcare providers, and administrative staff. Despite the remote nature of the work, you will have ample opportunities for engagement and support through virtual meetings and outreach efforts.

Key Responsibilities

• Initiate discharge planning for all patients on their admission day, ensuring a seamless transition and continuity of care.
• Conduct discharge sessions to teach patients coping skills and to reinforce understanding of their transition plans and available resources.
• Confirm discharge appointments with patients and their families, facilitating a supportive environment for post-treatment care.
• Maintain continuous communication with referral sources and outpatient providers throughout the patient’s treatment journey, from admission to discharge.
• Facilitate transfers to higher levels of care when necessary, coordinating with involved healthcare staff to provide the best possible patient support.
• Actively engage in clinical outreach initiatives and program tours, promoting awareness and advocacy for mental health services.
• Manage crises effectively during therapy sessions and within the practice setting, leveraging de-escalation techniques and best practices.
• Support milieu staff in their interactions with patients and families, contributing to a nurturing therapeutic environment.
• Collaborate with the treatment team to uphold the highest standards of clinical quality and effectiveness in therapy.
• Complete thorough clinical documentation, including initial assessments, treatment plans, and case notes in accordance with regulations.

Patient Population / Client Focus

The patient population served by our clinic is diverse, encompassing individuals from various backgrounds and with varying mental health issues. As an LCSW, you will encounter clients experiencing anxiety, depression, trauma-related disorders, and a spectrum of other challenges. Your role will involve understanding and respecting the unique cultural and socio-economic factors that influence each client’s mental health experience.

Qualifications

• A Master's degree in Social Work, Counseling, Psychology, or a related mental health discipline.
• Independent licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Proficiency in documentation and therapeutic reporting within Electronic Health Record (EHR) systems and Office 365.
• Demonstrated ability to set and maintain appropriate professional boundaries with clients.
• Strong written and verbal communication skills for effective patient engagement and collaboration with colleagues.

Preferred Experience

• Experience in crisis intervention and management, especially in therapeutic settings.
• Background in community mental health and familiarity with working in a health clinic environment.
• Experience developing SMART treatment plans and conducting initial patient assessments is highly desirable.

• Competitive annual salary ranging from $120,000 to $130,000 for full-time employees.
