Key Responsibilities

• Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental challenges related to their health conditions.
• Develop, implement, and evaluate individualized treatment plans using evidence-based practices, ensuring alignment with multidisciplinary team goals.
• Facilitate effective discharge planning by coordinating with healthcare professionals, insurance providers, and community resources to ensure a seamless transition from inpatient care.
• Provide education and counseling on coping strategies related to illnesses and life changes, empowering patients and their families to manage their unique situations.
• Utilize various therapeutic approaches, including crisis intervention, brief therapy, and family therapy, tailored to the specific needs of patients.
• Act as a resource and mentor to other staff members, offering insights into psychosocial practices and professional development.
• Participate in on-call rotation to address urgent after-hour patient needs effectively.
• Monitor and evaluate the psychosocial impact of cancer diagnoses on patients, offering appropriate interventions for anxiety and depression.
• Present at educational conferences as a subject matter expert in psychosocial oncology, sharing knowledge with peers and enhancing community resources for cancer survivors.
• Engage in community outreach through educational initiatives and collaborations with local organizations to strengthen available services for patients.

Patient Population / Client Focus

You will primarily work with patients and families facing mental health challenges associated with chronic illnesses, particularly in oncology settings. This includes addressing issues such as anxiety, depression, and adjustment disorders in patients undergoing treatment. Your efforts will support not only their clinical needs but also foster resilience and coping strategies that enhance their overall quality of life.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current and valid LICSW licensure in Massachusetts.
• Strong understanding of psychosocial and behavioral health principles.

Preferred Experience

• A minimum of three years of experience in a clinical social work role, preferably in a healthcare or community health setting.

Compensation and Benefits
