Work Environment

This position is fully remote, allowing for flexible working conditions while still preserving a strong connection to our clientele based in Valley Stream, New York. As a Social Worker at our hospice center, you will collaborate closely with a multidisciplinary team of healthcare providers, including nurses and medical staff, utilizing telehealth technologies and digital communication tools to engage effectively with clients and their families.

Key Responsibilities

• Conduct comprehensive evaluations of seniors' psychosocial needs, taking into account mental, emotional, and social factors that affect their care.
• Work collaboratively with clients and their families to create tailored care plans that align with their preferences and life goals.
• Provide emotional support and counseling to clients and their families, guiding them through various life transitions and challenges.
• Advocate for clients’ rights and access to community resources, ensuring they receive necessary services and supports.
• Regularly assess client living conditions and well-being through virtual visits, identifying and addressing any emerging concerns.
• Facilitate mediation among family members to resolve care conflicts and maintain a harmonious caregiving environment.
• Educate clients and families on available social services, support programs, and community engagement opportunities that promote well-being.
• Maintain detailed records of client assessments, care strategies, and interactions in compliance with professional standards and regulations.
• Stay updated on current social work methodologies and framework to ensure effective, ethical practice and service delivery.

The primary focus of this role will be on seniors receiving hospice care, a demographic that often faces complex emotional and psychological challenges. Qualifications

• Master's degree in Social Work (MSW) from an accredited institution is essential.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
• Minimum of two years’ experience in social work, ideally within hospice, palliative care, or a geriatric setting.
• Deep understanding of social work principles and aging-related issues, alongside familiarity with local community resources.
• Must demonstrate strong assessment, counseling, and advocacy skills with a genuine capacity for empathy and emotional support.
• Excellent communication and collaboration abilities, particularly in interdisciplinary team settings.
• Proficient in using Electronic Health Record (EHR) systems and standard office software.

Preferred Experience

• Experience working in a remote environment or using telehealth platforms.

Compensation and Benefits

We offer a competitive salary range of USD 60,000 – 100,000 per year, commensurate with experience and qualifications.
