Key Responsibilities

• Conduct thorough assessments and evaluations of patients' psychosocial needs, leading to customized treatment plans that address their unique circumstances.
• Implement evidence-based therapeutic interventions, including solution-focused therapy and grief counseling, to support individuals in navigating emotional challenges.
• Maintain detailed, accurate, and confidential client records, ensuring compliance with regulatory standards and organizational policies.
• Collaborate closely with a multidisciplinary team, including physicians, nurses, and chaplains, to coordinate comprehensive care strategies tailored to each patient's needs.

Patient Population / Client Focus
