Key Responsibilities

• Conduct detailed assessments to evaluate the emotional and psychosocial needs of patients and families.
• Develop individualized treatment plans that address the unique challenges presented by chronic illness.
• Implement evidence-based therapeutic interventions to support positive outcomes, utilizing techniques such as cognitive-behavioral therapy and solution-focused brief therapy.
• Maintain comprehensive and accurate documentation of patient interactions and treatment progress in a secured electronic health record system.
• Collaborate with a multidisciplinary team, including doctors, nurses, and other social workers, to coordinate the overall care for patients.
• Stay informed of the latest research and best practices in hospice care and LCSW interventions.

Patient Population / Client Focus

As a Licensed Clinical Social Worker at our hospice center, you will work closely with patients experiencing life-limiting illnesses, as well as their families. Your role will include guiding patients through emotional distress related to trauma, anxiety, and depression, often exacerbated by their health conditions. Providing supportive counseling and resources for coping strategies will be central to your practice.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• An active and unrestricted LCSW license in the state of Nevada.
• Exceptional communication skills, both verbal and written, to effectively convey complex information to patients and families.
• Strong problem-solving abilities and a compassionate approach to care.
• Proficiency in using telehealth systems and electronic health record platforms to facilitate patient communication and documentation.

Preferred Experience

Compensation and Benefits
