Key Responsibilities

• Directly engage with assigned clients through virtual platforms to assess their mental health needs and provide effective interventions.
• Develop and implement individualized care plans in collaboration with clients, their families, and other care team members, ensuring all services are integrated and accessible.
• Conduct thorough client assessments, utilizing evidence-based screening tools to identify areas requiring intervention and support.
• Maintain detailed case records and ensures timely documentation of client interactions, treatment goals, and progress updates.
• Participate in multidisciplinary team meetings to discuss treatment approaches and refine strategies based on the changing needs of clients.
• Provide guidance and crisis intervention when necessary, employing de-escalation techniques and supportive counseling to manage urgent situations.
• Lead brief, focused therapeutic sessions with individuals, groups, and families to facilitate understanding, coping strategies, and recovery.
• Actively contribute to quality improvement initiatives within the organization that enhance service delivery and patient outcomes.
• Remain abreast of best practices and advancements in the field, integrating new knowledge into daily practices.

Patient Population / Client Focus

Your core responsibilities will revolve around clients within a hospice care setting, many of whom are dealing with major mental illnesses or severe emotional challenges associated with terminal diagnoses. You will serve a diverse demographic, tailoring your approach to respect varied cultural backgrounds, beliefs, and individual experiences. Qualifications

• Active and unrestricted Licensed Clinical Social Worker (LCSW) certification in North Carolina.
• Bachelor's Degree in Social Work or a related field; a Master's Degree in Social Work (MSW) is preferred.
• Strong knowledge of mental health disorders, assessments, and treatment methodologies.
• Proficiency with telehealth technology and a strong aptitude for engaging clients in a virtual environment.
• Exceptional interpersonal, communication, and organizational skills, with a proven ability to work collaboratively in a team.

Preferred Experience

• Prior experience in a hospice, palliative care, or similar healthcare setting is strongly preferred.
• Experience working with clients from diverse backgrounds and with varied cultural contexts.
• Demonstrated skills in crisis intervention and treatment planning.
• Familiarity with community resources and referral processes to support clients post-treatment.

Compensation and Benefits

This full-time position offers a competitive salary in the range of USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
