Work Environment

This position is primarily remote, allowing for flexibility and work-life balance while serving a vital role in supporting our clients. Standard work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with potential evening or weekend hours as needed to accommodate the unique requirements of our clients. The effective social worker in this role must possess a proactive approach to scheduling, ensuring accessibility for our clients and continuity of care. There may also be occasional on-call responsibilities to support urgent situations during off-hours.

Key Responsibilities

• Perform comprehensive assessments of clients’ psychosocial needs, utilizing evidence-based approaches to consider emotional, mental, and social factors that impact their well-being.
• Collaborate closely with clients and their families to design personalized care plans that align with their goals, preferences, and circumstances.
• Provide counseling and emotional support to clients, helping them navigate significant life transitions, challenges, and foster resilience.
• Advocate for clients’ rights, ensuring they have access to necessary community resources and services tailored to their individual needs.
• Work in tandem with interdisciplinary teams of healthcare professionals, including nurses and caregivers, to enhance coordinated care.
• Regularly conduct client visits to monitor well-being, assess living conditions, and proactively address any safety concerns.
• Facilitate effective communication among family members involved in the care process, resolving conflicts constructively.
• Educate clients and their families regarding available community resources, support groups, and programs to enhance social engagement and overall quality of life.
• Maintain accurate documentation of client assessments, care plans, progress notes, and all relevant interactions within electronic health record (EHR) systems.
• Stay informed about current social work practices, ethical standards, and regulations to ensure compliance in all work-related activities.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work role, especially within a community center or geriatric setting.
• Strong understanding of social work principles, aging-related issues, and community resource networks.
• Exceptional assessment, counseling, and problem-solving skills.
• Ability to empathize and establish a strong rapport with clients and their families.
• Proficient communication and interpersonal skills, enabling collaboration with interdisciplinary teams.
• Familiarity with electronic health record (EHR) systems and computer applications for documentation and reporting.

Compensation and Benefits
