Key Responsibilities

• Conduct comprehensive clinical assessments to understand the emotional and psychological needs of patients and families within the context of hospice care.
• Develop individualized treatment plans that utilize evidence-based, solution-focused strategies to address the complex grief and mental health challenges faced by our client population.
• Implement various therapeutic interventions, such as individual therapy sessions, family counseling, and grief support workshops, tailored to the distinct circumstances of each case.
• Maintain meticulous and timely documentation of clinical records to ensure compliance with healthcare regulations and to support high standards of patient care.
• Work collaboratively with a multidisciplinary team, including nurses, physicians, and chaplains, to create cohesive care plans that address physical, emotional, and spiritual needs.
• Engage in regular case consultations and participate in peer review sessions to continuously elevate the standards of care provided.
• Educate patients and families on coping strategies and available resources to empower them through the grieving process and mental health challenges.
• Stay abreast of current research and best practice guidelines in social work and hospice care, integrating new knowledge into your practice to improve patient outcomes.

Qualifications
