Remote Licensed Clinical Social Worker (LCSW) for Hospice Care

Key Responsibilities

As an LCSW at Essential Grace Mental Therapy, you will be responsible for:

• Conducting thorough clinical assessments to understand the unique mental health needs of patients in hospice care, followed by the development of tailored treatment plans that incorporate evidence-based interventions.
• Administering therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), ensuring interventions are responsive to the individual’s circumstances and preferences.
• Maintaining accurate and up-to-date clinical documentation in accordance with HIPAA regulations, ensuring compliance and fostering quality care.
• Participating in multi-disciplinary treatment planning and case meetings with psychiatrists, psychologists, and other healthcare providers to facilitate coordinated, client-centered care.
• Educating patients and families on mental health issues, treatment options, and community resources, fostering informed decision-making.
• Staying current on clinical best practices and integrating new research findings into your treatment methods to continuously improve quality of care.

Patient Population / Client Focus

Your primary focus will be on a diverse hospice patient population, encompassing individuals and families facing end-of-life issues. You will work closely with those dealing with chronic illness, emotional distress, and the grief process, utilizing a compassionate and skilled approach to enhance comfort and quality of life.

Qualifications

To be successful in this role, candidates must possess:

• A Master's degree in Social Work from an accredited institution.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.
• A minimum of three years of clinical experience, with a solid background in providing psychotherapy services.
• Demonstrated knowledge of solution-focused psychotherapy techniques and approaches.
• Strong interpersonal and communication skills, allowing you to engage effectively with various stakeholders.
• Robust problem-solving and critical thinking abilities, enabling you to navigate complex patient scenarios.

Preferred Experience

While not essential, experience in hospice care or palliative support, as well as familiarity with working in a remote environment, is highly advantageous and will be considered during the application process.

Compensation and Benefits
