Key Responsibilities

• Provide direct clinical services to assigned clients experiencing major mental health conditions, focusing on evidence-based treatment modalities.
• Conduct thorough assessments, develop personalized treatment plans, and implement interventions that align with client goals.
• Engage with clients in both scheduled appointments and crisis situations, providing crisis management as needed.
• Document and maintain accurate client records, ensuring compliance with ethical standards and organizational policies.
• Work collaboratively with a multidisciplinary team to foster the best outcomes for clients, exchanging ideas and models of support.

Patient Population / Client Focus

Your main focus will be on individuals who suffer from severe emotional disorders and major mental illnesses. Working within the hospice setting, you will engage with clients at various stages of their treatment, addressing both immediate mental health concerns and long-term care planning. By understanding the unique challenges faced by our patient population, you will contribute to holistic care designed to empower clients and improve their quality of life.

Qualifications

• A valid LCSW license is required to qualify for this role.
• A Bachelor’s degree in Social Work or a related field is essential.
• Strong interpersonal skills, enabling respectful collaboration with colleagues and clients from diverse backgrounds.
• Exceptional verbal and written communication skills to facilitate meaningful therapy sessions and team interactions.

Preferred Experience

• A minimum of 2 years of experience as a clinician, preferably in a mental health setting.
