Key Responsibilities

• Conduct thorough evaluations and psychosocial assessments of clients to identify their needs and develop personalized intervention plans.
• Engage in individualized treatment planning and participate in Individualized Family Service Plan (IFSP) development, ensuring all family members are included in the care process.
• Collaborate with interdisciplinary team members to monitor and evaluate client outcomes, adjusting treatment plans as necessary.
• Provide consistent support and counseling to families to promote understanding and facilitate the well-being of seniors.
• Ensure compliance with local, state, and federal regulations while adhering to best practices in clinical social work.
• Build and maintain trusting relationships with seniors and their families, promoting a positive and healthy support system.
• Administer and interpret assessment tools and strategies to track client progress effectively.

Patient Population / Client Focus

Your primary focus will be on seniors, a growing demographic that often requires specialized support in managing both mental health challenges and the complexities of aging. You will work closely with individuals facing various life transitions, including changes in health status, loss of independence, and bereavement. Qualifications

• A Master's Degree in Social Work (MSW) from an accredited institution is required.
• Current LCSW licensure in the state of Texas is necessary to practice.
• Exceptional communication skills, both verbal and written, to effectively interact with a variety of stakeholders, including clients, families, and colleagues.
• Strong organizational and planning abilities to efficiently manage multiple cases while adhering to deadlines.
• Demonstrated understanding of assessment, diagnosis, and treatment methodologies pertinent to the senior population.
• Experience working with clients facing age-related challenges and mental health issues.

Preferred Experience

• A minimum of two years of clinical experience directly working with seniors in a mental health or community health setting is preferred.

Compensation and Benefits
