Licensed Clinical Social Worker for Hospice Care

Position Overview

We are seeking a passionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team at The Center for Healing and Growth in Overland Park, Kansas. In this full-time role, you will provide vital psychosocial support and counseling services to seniors and their families in a remote capacity, fostering their well-being and improving their quality of life. This position offers a competitive annual salary ranging from USD 60,000 to 100,000, depending on experience and credentials.

Work Environment

This role is designed as a remote position, providing flexibility while allowing you to serve clients effectively. Although your main tasks will be conducted virtually, you will also need to make occasional home visits to assess client needs and conditions. Key Responsibilities

• Conduct in-depth assessments of clients’ psychosocial needs, taking into account their emotional, mental, and social backgrounds.
• Develop and implement individualized care plans in collaboration with clients and their families, tailored to their unique challenges and desires.
• Provide therapeutic counseling to clients, offering support through significant life transitions and complex challenges.
• Act as an advocate for clients, ensuring they have access to necessary community resources and rights to facilitate their care.
• Regularly communicate with interdisciplinary teams—including healthcare professionals, nurses, and caregivers—to enhance integrated care approaches.
• Facilitate periodic home visits to assess clients’ living environments and address identified issues that may impact their well-being.
• Help mediate and resolve conflicts that may arise among families involved in caregiving, fostering a supportive family dynamic.
• Educate clients and their family members about available resources, support groups, and community programs that can enrich their lives.
• Maintain accurate, detailed documentation regarding assessments, care plans, progress notes, and other professional interactions.
• Stay current with best practices, evolving social work policies, and ethical standards to ensure the highest quality of care.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• A deep understanding of aging-related issues and social work principles.
• Exceptional assessment, problem-solving, and counseling abilities.
• Demonstrated empathy and the ability to build rapport with seniors and their families.
• Strong communication and teamwork skills for effective collaboration.

Preferred Experience

• Experience in hospice or palliative care settings.
• Advanced knowledge of community resources specific to the aging population.
• Skills in mediation and conflict resolution, particularly within family dynamics.
