Work Environment

This position offers a 100% remote work environment, allowing for a flexible work-life balance while serving a critical population. Although much of your work will be conducted from home, you will also maintain regular interactions through virtual platforms with clients, families, and healthcare professionals. This role is structured around a Monday to Friday schedule, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required based on client needs.

Key Responsibilities

• Conduct comprehensive psychosocial assessments for seniors to identify their emotional, mental, and social needs, tailoring interventions accordingly.
• Collaborate closely with clients and their families to create and implement personalized care plans that encompass their specific goals and desires.
• Provide direct counseling services to seniors and their families, emphasizing emotional well-being and effective navigation of life's transitions.
• Advocate tirelessly for clients, ensuring they have access to the necessary resources such as community services, support groups, and social engagement opportunities.
• Engage with interdisciplinary teams—including nurses and healthcare professionals—to coordinate care efforts, contributing to a holistic approach to client well-being.
• Conduct routine check-ins to monitor clients' emotional and social health, report observations, and make necessary adjustments to care plans.
• Facilitate productive communication among family members, helping to mediate any conflicts and ensuring cohesive care strategies are maintained.
• Stay current with social work best practices, policies, and ethical standards to maintain compliance and deliver the highest quality services.

Patient Population / Client Focus

Your primary focus will be the senior population within our community, emphasizing their unique challenges, including social isolation, health transitions, and emotional distress. You will be instrumental in improving their quality of life through individualized support and systemic advocacy, ensuring they have the tools and resources necessary for a fulfilling life as they age.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid and current state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of relevant experience within social work, preferably in settings addressing the needs of seniors or in community-centered roles.
• Strong understanding of social work principles, aging-related issues, and community resource networking.
• Exceptional skills in assessment, counseling, and problem-solving, with a demonstrated ability to build rapport with clients.
• A commitment to continuous professional development and ethical practice.

Preferred Experience

• Experience working directly with the elderly population, particularly within community health settings or geriatric care.
• Familiarity with electronic health record (EHR) systems and other relevant documentation practices.
• Proven ability to work collaboratively in an interdisciplinary team environment.

Compensation and Benefits
